There are two ways to get the fluoroscopy license renewal form.
Option 1: You normally receive your fluoroscopy license renewal form about 3 months before your license expires. The form arrives at the address that you registered with the RHB the last time you renewed.
Option 2: Email support@orbitcme.com and our team will reply with an electronic version of the form.
