How to update your Fluoroscopy license mailing address

The RHB sends your fluoroscopy renewal paperwork the the address registered with your license. Keep your address accurate so that you can easily renew your license every 2 years.

How do I change my address on my California fluoroscopy license renewal paperwork?

On the renewal application sheet the RHB sends you when it is time to renew your license, you are able to update your address if you know you will be moving in the next 2 years or if you want to switch addresses for another reason. If you want to change the address registered with the RHB from the one listed in the the red you will write the new address on the lines under "Enter correct information below" and check the "no" box above the red box.

What if I need to change my address and I don't have my renewal paperwork?

If you have moved during the 2 years your license is active and need your renewal paperwork sent to a new address or need to change your address for another reason before your next renewal, you can submit this form to the RHB.

You can fax the form to 916-440-7999, which is easiest.

What if I need a copy of my fluoroscopy license with the new address?

If you would like a copy of your fluoroscopy license with your updated address you need to include a check for $2.00 payable to the CDPH-RHB with your form when you mail it.
